1802102016 final practical-Gm short case

  This is an online e log book to discuss our patient de-identified health data shared after taking his /her /guardians signed informed consent.Here we discuss our individual patients problem through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evidence based input.

A 65 YEAR OLD MALE TODDY TREE CLIMBER BY OCCUPATION CAME WITH C/O ANURIA SINCE 2 DAYS.

History of present illness:- 

-Patient was apparently asymptomatic 15yrs back ,then he fell down from toddy tree and had a history of fracture of femur for which surgery was done .

- Again 10 yrs back patient fell down due to giddiness .

- Patient also diagnosed with hypertension and on regular medication .

-Patient has got flexion fixid deformity 7 yrs back and he stopped climbing tress and stays at home .

-Patient came with anuria since 2 days and SOB .

-No case of palpitation ,chest pain ,syncope attacks .

Past history :- 

No history of diabetes,CVA,CAD,Asthma , epilepsy .

Treatment history:- 

 Tab Amlong 5mg PO/OD for hypertension 

History of surgery for renal caliculi 

Personal history :- 

Diet :- mixed 

Appetite :- decreased 

Bowels :- irregular 

Micturition :- abnormal


Addictions:-occasional alcohol consumption ,has history of smoking .

General examination :- 

Patient is conscious, coherent, cooperative and well oriented to surroundings.

Haa pallor and  edema .







Vitals :- 
TEMPERATURE - 98.6 F
PULSE - 100 BPM
RR - 16 CPM
BP - 160/90 mmHg
Spo2 - 98 % @ ROOM AIR
GRBS: 106MG%

Systemic examination :- 

CVS :- S1,S2 heard ,no murmurs 

Respiratory system:- Dsypnea present ,breath sounds vesicular .

CNS :- Intact 

Abdomen :-  soft ,non tender .

Investigations :- 




 

 









Provisional diagnosis :- 
AKI secondary to ? Acute urinary retention.


Treatment:- 

Inj. Meropenem 500mg / IV/BD

2) Inj. LASIX 40mg/IV/BD

3 ORAL FLUIDS ~2L.

4) Inj. PAN 40mg /IV/OD

5) Neb with DUOLIN & Budecort 8th hourly

6) Tab. AMLONG 5mg po/OD
